Forms for city-sponsored events.
Businesses in Clay, do business in the city of Clay or with the city of Clay must have a Clay business license. See the ordinance below. Both the Estimated Gross Receipts Form and the Business License Application must be completed and turned into city of Clay administration.
Any business that pays sales tax and is located in the city of Clay or conducts business in the city of Clay must remit city of Clay sales tax payments to RDS/ALATAX. Due to the fact that the city of Clay does not have its own ZIP code, you must specifiy "City of Clay" with jurisdiction code 9767 on the remittance form.
Any business that is a contractor or vendor of the city of Clay must fill out the Affidavit Concerning Unauthorized Aliens and have it notarized. Additionally, any contractor or vendor of the city of Clay must fill out the IRS W-9 Form.
All forms must have hand-written signatures.
